Please prepare and provide the following documentation:

Certified Birth Certificate or County Recorder's Certificate of Live Birth
Current immunization records
Proof of Address (mortgage/rental agreement or utility bill)
Parent/Guardian Identification
Completed Oral Health Exam (TK, Kinder, and 1st grade only)
Legal custody paperwork, if applicable

Required documents can either be uploaded during the Pre-Enrollment process, or you can bring physical copies into the office when completing enrollment and we can help upload them.
